Back to Work Enterprise Allowance Participant Details Forms


The Back to Work Enterprise Allowance scheme is co-funded by the European Union for participants who are under the age of 30, through the European Social Fund Plus (ESF+ 2021-2027). The ESF+ is the EU’s main instrument for investing in people, including skills, education and addressing disadvantage. It aims to improve employment and education opportunities, to enhance social inclusion and to tackle poverty.


Participant Forms

All participants under the age of 30 when they commenced the Back to Work Enterprise Allowance are being issued with 3 separate Participant Forms by email as outlined below. These forms request information which is needed to meet EU funding and reporting requirements under the European Social Fund Plus (ESF+ 2021-2027).

All forms should be responded to within 10 days of receiving the email.

Stage 1 form is issued while you are in receipt of the Back to Work Enterprise Allowance.

Stage 2 form is issued approximately 4 weeks after you have completed the Back to Work Enterprise Allowance.

Stage 3 form (final) is issued approximately 6 months after you have completed the Back to Work Enterprise Allowance scheme.


Stage 1 – Participant Questionnaire

All participants under the age of 30 when they commenced the Back to Work Enterprise Allowance are being issued with the attached Participant Details Form (see below). This form requests additional information needed to meet EU funding and reporting requirements under the European Social Fund Plus (ESF+ 2021-2027).

The form is in PDF format and can be edited. We would ask that you complete the form in full and return by email within 10 days of receiving this notification.

You will be asked to complete 2 further forms at a later stage in the process. We appreciate your co-operation. If you have any queries, please contact our team at the email address.

Stage 2 – 4 week post questionnaire

As previously advised all participants who were under the age of 30 when they commenced the Back to Work Enterprise Allowance are being issued Participant Details Forms (see 4-week post questionnaire below – stage 2). This form requests additional information which is needed to meet EU funding and reporting requirements under the European Social Fund Plus (ESF+ 2021-2027).

The form is in PDF format and can be edited. We would ask that you complete the form in full and return by email within 10 days of receiving this notification.

You will be asked to complete a final form at a later stage in the process. We appreciate your co-operation. If you have any queries, please contact our team at the email address.

This form requests additional information which is needed to meet EU funding and reporting requirements under the European Social Fund Plus (ESF+ 2021-2027).
