On 25 March 2021, Sláintecare hosted the 8th Right Care, Right Place, Right Time, Webinar - ‘‘Alternative Pathways”
This webinar heard from Martin Dunne, Director National Ambulance Service, who spoke about alternative pathways being delivered by the National Ambulance Service, to provide more care in the community.
This webinar shared stories from Sláintecare funded projects which are providing alternative care pathways for patients. These projects are contributing to hospital avoidance and improving the experience of people who use our health and social care services.
Integrated Care for Older People Model for falls prevention and management (Project ID 173)
This project aims to implement an Integrated Model for Falls Prevention and Management in CHO 1. This is a joint integrated venture between CHO 1 and Sligo University Hospital (SUH) in SAOLTA Hospital Group.
The innovative nature of this project is a holistic approach to falls prevention and management incorporating a multi-disciplinary team of a senior Physiotherapist and two therapy assistants, a Clinical Nurse specialist and clerical support with supporting clinical leadership by an existing Geriatrician, advanced nurse practitioner (ANP) and integrated care of older persons team. This team will provide rapid access to specialist falls gerontology services for GPs and primary care health professionals. It will aim to reduce emergency attendance to acute hospital by having the availability of the specialist service in the community and reduce length of stay by having an onward referral pathway.
COPD Integrated Care Project (Project ID 164)
This COPD Integrated Care Project aims to address identified priority service gaps in Waterford Respiratory / COPD Services including the need for an integrated community-based approach to care.
The project aims to improve the quality of life of patients with COPD within an integrated service delivery approach, through timely, efficient and accessible healthcare and support for COPD patients, delivered mainly in an appropriate primary care setting and hospital avoidance and supported discharge through tailored transition strategies and follow up care.
CIT Dublin North City and County Community Oncology Service (Project ID 247)
The project will provide expert care to cancer patients outside the hospital setting. This will enable expansion of the Community Intervention Team (CIT) Community Oncology Service to increase the number and type of treatments it can deliver to patients in the CIT clinic, in collaboration with the Mater Misericordiae University Hospital (MMUH). This will in turn reduce pressure on acute hospital Cancer Care Directorate services. The aim of the project is to provide expert care to cancer patients outside the hospital setting.
Integrated care for patients presenting with leg ulcers in Dublin South, Kildare and West Wicklow (Project ID 375)
This project will establish a collaborative service to facilitate leg ulcer clinics in primary care centres, led by Tallaght University Hospital in partnership with Community Healthcare Dublin South, Kildare and West Wicklow. The service will enable integrated care between the community and hospital to be further developed in order to improve the care of leg ulcer patients.
