To make a Subject Access Request (SAR), please download the Subject Access Request Form and email the completed form to or post to:

Required documents
Requests for personal data must include:
- your name and postal address
- a photocopy of your passport or driving licence or Public Services Card to verify your identity
- proof of address such as a utility bill or Government issued letter
- your signature and the date of the request
- if you are applying on behalf of another person, then a signed authority from the individual is required
- any details which may help the Department locate the information which is the subject of your request, such as reference numbers or area of the Department you were in contact with
