Facts/brief background

1. Mr. [ ] ('the Applicant') has made a claim for compensation under the Scheme of Compensation for Personal Injuries Criminally Inflicted ('the Scheme').

2. The application arises pursuant to an incident which occurred at [ ] on the [ ].

3. In his application to the Tribunal the Applicant states he was assaulted and suffered the loss of [ ] teeth. The Applicant attended [ ] Hospital, and for dental treatment as a result of his injuries.


Eligibility

4. The Tribunal acknowledged receipt of the application by way of letter dated the [ ], and requested vouching documentation, a copy of the Applicant's P60 and a statement from the Department of Social Protection, no response was received thereto. On the [ ] the Tribunal wrote to the Applicant by way of correspondence asking him to confirm whether garda proceedings had concluded, no response was received thereto. On the [ ] the Tribunal again wrote to the Applicant, no response was received thereto. Finally, on the [ ] the Tribunal again wrote to the Applicant advising that if no response was received the matter would be passed to the Tribunal member for the issuing of a decision.

5. In the absence of the provision of sufficient information by the Applicant no award is made pursuant to paragraph 10 (previously 11) of the Scheme.
