On 15 May 2017 His Honour Judge Donagh McDonagh was appointed to conduct an inquiry under section 42 of the Garda Síochána Act 2005 (as amended) to inquire into the conduct and adequacy of the investigations conducted by the Garda Síochána into the death of Mr. Shane Tuohey in February 2002 in Clara, County Offaly.

Upon the retirement of His Honour Judge Donagh McDonagh, the Minister for Justice appointed Judge Michael Coghlan, retired judge of the District Court, to continue the work of the Inquiry on 11 January 2022.
